Making A Request

Important things to note:

  1. Forms should have sufficient information so that records can be easily identifiable
  2. You must complete all requested information such as name, date and signature before submitting forms
  3. If the form is hand delivered you should record date and time of delivery, the name of the person that received the form on behalf of the authority
  4. For third party, mail or courier deliveries, you should record the date, time and mailing location
